A 2018 Ford Escape, driven by Dennis Heins, 76, of Spencer, was stopped at the stop sign at the intersection of Fourth Avenue West and West 18th Street, when Andrea Dailey, 71, of Spencer, traveling west through the intersection on her bicycle.

Heins stated he did not see the bicycle in the crosswalk and struck the bicycle traveling north in the crosswalk.
